About the Webinar

The nation is currently undergoing a transition to a value-based healthcare system with a primary care foundation and population health as an organizing principle. Research and experience shows that a successful approach to these new models of care include a shift away from physician centered care delivery towards a team-based approach. However, the implementation of team-based care has a fundamental impact on workflow at the micro, or practice level as well as the macro, or system and network levels. Understanding and orchestrating this impact and standing up teams that reflect the needs of the population are critical to winning at value based contracts.
